Description: Urban League of Rochester, NY, Inc. Programs Serving the Elderly; Expanded In-Home Services for the Elderly Program (EISEP) The Urban League of Rochester is part of the EISEP Community Collaboration with Catholic Family Center as the lead agency. Through this collaboration, the Urban League of Rochester provides case management to individuals over the age of 60 living in Monroe County who have functional limitations and are not eligible for a similar type of service through another funding source such as Medicaid. The Urban League focuses on serving the elderly who are minorities. Through the program, individuals receive subsidized in-home services that enable them to live independently and safely in their homes for as long as possible. Baldwin Richardson Senior Support Group The Urban League of Rochester's Baldwin Richardson Senior Support group is a monthly support group for individuals 55 and older who live in Monroe County. It is targeted at African-American seniors living in the city of Rochester. Each meeting includes lunch, group discussion, and presentations from guest speakers. In addition, program staff assess participants' needs for community services and link them to appropriate services. Kinship Care The Urban League of Rochester's Kinship Care program serves low-income families in which the children reside with a grandparent or other relative caregiver. A case manager provides specialized community outreach and school-based support services to the caregivers and children. Through assessment and identification of each family's needs, the case manager links the caregivers and children to the needed services within the community and the schools. The case manager advocates for the family when needed.
